Laboratory Science Can Only Analyze What It Receives.
Modern forensic laboratories use highly sophisticated instrumentation to measure ethanol concentrations in blood. Instruments are calibrated using certified reference materials and quality control samples to ensure accurate measurement of alcohol concentrations.
These laboratory procedures are well established and scientifically rigorous. However, even the most precise analytical methods cannot correct for a blood sample that was improperly collected or contaminated before arriving at the laboratory. Simply put: A laboratory cannot produce a scientifically reliable answer from an unreliable sample.

The preanalytical phase includes everything that happens before laboratory analysis, including:

Why Proper Sterilization Matters
Blood is an exceptionally complex biological material containing cells, proteins, carbohydrates, lipids, and electrolytes. Before a blood draw, accepted forensic protocols recommend properly disinfecting the collection site with an appropriate antiseptic solution and allowing adequate contact time to reduce microbial contamination. After the site has been sterilized, it should not be touched again. Proper hand hygiene and sterile gloves are equally important.
Yes—Blood Can Produce Alcohol After Collection
One of the lesser-known scientific issues in BAC testing is post-collection fermentation. Blood naturally contains sugars that can serve as nutrients for yeast and bacteria. If contamination occurs, microorganisms may convert those sugars into ethanol after the blood has been drawn. Laboratory instruments cannot distinguish between ethanol produced by drinking alcohol and ethanol produced by microbial fermentation after collection.
Why Grey-Top Tubes Matter
Blood alcohol testing requires specialized grey-top collection tubes containing potassium oxalate and sodium fluoride. These tubes should be filled to the recommended volume, properly mixed through complete inversions, and stored appropriately after collection. Underfilled tubes or improperly mixed preservatives may reduce preservation and increase the risk of sample degradation.
Temperature Control Is Critical
Once collected, BAC samples should be refrigerated promptly. Cool temperatures slow bacterial growth, enzyme activity, and chemical changes within the sample. Leaving blood samples unrefrigerated may increase the risk that the sample no longer accurately reflects the individual's blood alcohol concentration at the time of collection.
